There's also a compromise between the two choices I mentioned - a combi
boiler with a 'large' thermal store.  Vokera make one

- will give 18 litres per minute with
a 60 litre storage capacity.  Might not be any good if you like long
showers
though.
We've got a 18 litre per minute hot water pump for our shower and it's
fairly powerful, but we've got a 230 litre cylinder of hot water at 65oC,
so
it's good for long showers.

BTW. whilst planning our house refurb and searching for energy
efficient
stuff I found a clever heat recovery system for drains.  The hot water
from
a shower just goes down the drain, but if you use a heat recovery drain
pipe
section to partially heat the cold water you can make a 10kW shower
into
something like a 15kW shower, or lower the flow rate and save energy. 
Many
large hotels use heat recovery systems - around 70% of the heat from
waste
water can be extracted.
