I'm assuming this is just a abstract statistics question, rather than
a safety-of-flight question.

Engine failure is just one of a complex set of possibilities that can
occur causing death by aircraft, and due to the unusual nature of the
distribution of probabilites of bad things happening, MTBF isnt a
useful measure for liklyhood of death.  Probability of death by
aircraft is quoted as a number in terms of MPD, million passenger
departures.
