Combining Two RF signals

My cable box has an RF output, but no RF in, so when it comes to
distributing my TV signal around the house, I need a way to combine the
aerial signal with the output from the cable box.  Does anyone know of a
device to do this?

My cable box output is on channel 65, and all my terrestrial signals are
below channel 40, so presumably they could be filtered and mixed somehow
- I just haven't been able to find any off-the-shelf device to do it...
