Thanks for the comments.  I've added some more pictures.  It looks like
the plug sustained very little damage so I'm guessing it might have
"started" at the appliance module though it's hard to see exactly
what
might have happened to it.  I've got quite a few of these appliance
modules so this has me a little concerned.

The appliance was still working and it was only chance that meant I
found this problem at all.
