I can't comment on the above as that's not how I my asterisk is set up.
Regardless, it's not related to axc.

That is correct assuming that you placed axc (and all of it's contents)
in /usr/share/axc.

It's *very* important to thoroughly read and understand the axc.conf
file as it defines the behavior of axc. It constitutes the current
state of documentation (which I realize is rather lacking). Do feel
free to suggest changes or ask questions either on- or off-list. For
one thing, axc only reports on combinations of * users and peers that
are defined in axc.conf. In addition, you must declare whether you are
running a local hub (personally, I use Patrick's hub--which is quite
nice) or want to listen on the xAP port and perhaps most importantly the
level of verbosity for the log file. I think that the default is set to
3. That's probably right for most folks (or lower it if everything is
always running well). I would suggest taking a look at what is in it as
it would give clues as to what is/is not working.
If it's at all a mystery, then please feel free to mail me your log and
I'll be happy to take a look. I would probably also need to understand
your * config--such as what devices and/or VoIP services you are trying
to use. If there is a problem, I'd probably need to look at your
sanitized sip.conf, iax.conf (if you use iax) and zaptel.conf (if you
use zap devices--which it appears from the above that you do).
Something to remember/realize is that the voicemail message reporting is
performed in a CTI2 schema as that's not been formally adopted (or some
variant) by CTI (which I probably need to revisit w/ Kevin). That's not
a problem for me as I have CTI2 listeners in my misterhouse config.
But, it could be problematic to get the info that you want out. I
haven't used James' floorplan for performing macro/mapping functions,
but that might work for you. Alternatively, I could consider sending
the data out in some form of a generic message.
