I had an additional phone line installed for ADSL the other week.  The
day it was installed, I placed the ADSL order (with another provider).
Two days later, BT refused to acknowledge receipt of the order from them
because the installation was "too new" and asked the ISP to resubmit
the ADSL order, which they did, immediately, and it was accepted.

(The engineer also wanted to use a DACS on the phone line because there
weren't any spare pairs in the cable run under the road.  I pointed out
that I'd quite clearly said the line was for ADSL when I'd ordered it and
that they'd only have to come out and remove the DACS.  The engineer told
me that's exactly what they do, even though they know ahead of time and
could save themselves a job, but in this case he was very helpful in
getting it sorted so that a second trip wouldn't be required.)
